  • Student School Notes:  Are you looking for an easy way to write your child a note for an absence, early dismissal, or other permissions?  Try this new WCS School Notes form.  The form prints 4 per page.
  • Internet Network Access Agreement:  This permission slip must be signed and returned to school prior to a students ability to use the internet.
  • Permission to Publish:  Occasionally we photograph children performing their everyday tasks, receiving awards or any number of activities.  Prior to posting pictures to our website we must have this signed form on file.
  • Volunteer Form:  We are very fortunate to have many parents/guardians volunteer at our school.  If you would like to volunteer (including attending field trips), please download, fill out and return this form to the office.
  • 2013-2014 School Choice Form:   Parents choosing to send their children to a non-community school must complete this application for the 2013-2014 school year and return it to the Superintendent’s Office, 34 Wing Farm Parkway, Bath, ME 04530. The deadline for applications is May 1st; decisions will be made by June 1st. Decisions will be made on a first come, first served basis, pending availability of grade level slots. A separate application must be completed for each child.
